Request a Transcript

To request your Fox College transcript, please fill out the form with your current address, what program you attended, and where the transcript should be mailed. We will be in touch or please call us at 708-444-4500.

There is a $5.00 fee for each transcript request. You have the option to pay online via credit card after you fill out the form. Alternatively, you may print out a copy of this form and mail it to us along with a $5.00 check or money order. We will not accept cash.
